Question:
I have pouches under my eyes that make me look tired all of the time. I read that this is fat. I would like blepharoplasty to remove this fat. Will all of the fat be removed? Is any of the skin removed?
Answer:
The pouches beneath your eyes is likely fat. The approaches to lower lid blepharoplasty have been modified over time as we have learned the importance of this lower lid fat and how it will affect the appearance of your face as you continue to age. I do not resect all of the fat of the lower lids. I may resect some of the fat, but, I generally reposition the fat to fill in the hollow areas of the lower lids often referred to as the tear trough deformity. Removing too much fat from the lower lid will give you a hollowed out appearance without fat bags, not a refreshed looking appearance. A complete exam of your lower eyelids will be necessary to assess whether or not skin needs to be removed from your lower lids at the same time as fat repositioning and removal. Skin removal from the lower lids is more common in an older population.
